Sample Submittal Form (Chain of Custody)
If you are planning to bring a sample to the lab for analysis, you will be asked to fill out a Sample Submittal Form. Or you may print a form and fill it out before you come to the lab to make your service even faster!

New Account Form
If we have not done business with either you or your company before, you will be asked for certain information in order to open your account. To expedite this process, print this information form and fill it out before arriving at the lab and we'll take care of the rest.

Billing Credit Application
New accounts are most commonly dealt with on a "Cash On Delivery" basis. If you would like to set up a billing account to avoid this hassle, print this credit application and fax or bring it in to the lab for approval. Our goal is to make your visit as hassle free as possible.
